Psychosocial Support Program- Miwatj Health

Commonwealth Psychosocial Support program provides case management support to help East Arnhem residents living with a mental illness to build their functional capacity to address their goals. Funding for the program is provided through the Northern Territory Primary Health Network (NTPHN) who are funded by the Department of Health.

Psychosocial support can be provided in the following domains:

• Developing social skills and friendships • Building relationships with family • Managing money • Finding and looking after a home • Building skills and qualifications • Developing work goals • Staying physically well, including exercise • Support with drug, alcohol and smoking issues • Building life skills, including confidence and resilience.

Eligibility Criteria: 
1. Applicant has or appears to have mental illness that impacts on daily life and functioning 2. Applicant is not a current participant of NDIS 3. Applicant is residing in the East Arnhem region 4. Applicant is aged 16 and over
Assessment Procedure: 
Assessment procedure: 1. Complete the form with all the requested details and signatures 2. Email the completed form to npsm@miwatj.com.au with the subject line as: ‘REFERRAL- CLIENT INITIALS’ OR hand in the completed form to a relevant Miwatj Team Member
