Psychosocial Recovery Coaching

Uses a person-centred, strengths-based approach to build strong relationships with participants. Recovery Coaches aim to help participants build long-lasting hope and increase their capacity for managing their own mental health journey. Recovery Coaching involves NDIS Support Coordination of both informal and formal supports. They help to facilitate the participant’s and relevant stakeholder’s understanding of the NDIS and utilisation of their NDIS funds.

TeamTALK

Psychosocial phone-based support line for Territorians.

Improve access to mental health services early in the trajectory of mental illness, or potential mental illness, to improve the likelihood of recovery as well as improved health outcomes

Provide Territorians with evidence based psychological intervention (eg cognitive behavioural therapy (CBT), assistance with navigating other low intensity mental health supports.

Complex Residential Support

Medium and long term, 24/7 complex residential support, within a recovery orientated framework and using a service delivery model based on holistic, psychosocial rehabilitation.

Assists individuals in stabilising their mental health in a safe holistic environment enabling transition to safe and independent living in the community.

Interim Program

Provides short term accommodation to people for up to three weeks.

Medium Term Accommodation (MTA)

Our MTA offers a safe place for NDIS participants to stay during a transitional circumstance (such as leaving hospital). Our recovery-focused Residential staff can support you to plan your next moe into more permanent accommodation, as well as support your psychosocial recovery goals.

MTA is provided in shared residential sites across Darwin, typically for up to 90 days.

Short Term Accommodation (STA)

Our STA offers a safe place to experience time away from their normal place of residence. We tailor your experience to suit your needs and can accommodate all requirements such as arranging and supporting appointments, social outings and family and friend visits, day-to-day living and self-care support and planning towards psychosocial recovery and NDIS goals.

STA is provided in shared residential sites across Darwin, typically for up to 14 days.

Supported Independent Living (SIL)

Our SIL provides you with all of your everyday support needs in a safe and comfortable shared living environment. our SIL staff use a person-centred approach to understand and help you plan your recovery journey, support you to build your independence and reach your goals.

TeamHEALTH is a registered NDIS Provider.

Mental Health Education

Trained facilitators provide up-to-date, evidence-based education and training for workplaces, schools and individuals in the community.

Community Wellbeing & Healing Program

The Community Wellbeing & Healing program provides culturally appropriate wellbeing and healing programs, and activities in partnership with Larrakia Nation. The community wellbeing and healing program is part of the Darwin –National suicide prevention trial. We work towards improving outcomes for individuals and families so they can feel supported and empowered to thrive within their community, improving sense of belonging within a positive community, and enriching social connectedness
